All competitive grant recipients must turn in a final report. Submit your final report in the Grant Portal.

Non-Competitive Grant Reporting

Final reports may be required for grants outside of our competitive processes. If your award letter noted that a final report is required, please address the following narrative and financial requirements in an email to Logan Heard. 

Narrative

Please tell us how this grant impacted your organization. 

Submit a brief narrative (500-1,000 words) detailing how and where grant funding was spent. Describe any challenges that were encountered, mid-course corrections, and successes that resulted. List all areas and populations that were impacted by grant funding. If available, provide any metrics or measurable outcomes that were recorded.

Financial Report

For financial documentation of expenditures please submit the following documentation. Acceptable formats for document submissions include PDF, Word, Excel, and JPEG. 

  • Fiscal Year Income statement during the time the grant was received. (Do not submit tailored or redacted statements.)
  • Summary statement listing all expenditures paid with grant proceeds.
  • Copies of all invoices paid with grant proceeds.
